Golden Heart Care offers a variety of residential/group home and outpatient services to members in our care. See below all services and evidence-based therapies we incorporate.
Group homes operate like traditional family homes, with up to 5 adult residents. GHC offers a variety of services to each individual we serve. Our services are tailored to meet the needs of the individual. Below you will find the services we offer! We are also working on opening up a facility that can service up to 10 members at a time.
With therapy services each member will receive the development of fine motor skills, self-help skills, self-care skills, communication skills, coordination and balance, socialization skills, sensory integration, and attention and problem solving skills needed for every stage of their life.

The Golden Heart Care Vocational Program provides a continuum of training and vocational opportunities in safe, supportive environments. Our vocational programs stress training in proper work habits and attitudes, functional academics, as well as modeling of on the job skills. Currently, individuals experience "hands on" job training while being supervised and assisted every step of the way.

Golden Heart Care offers Case Management services to all members while in serve in one of our Behavioral Health Residential Facilities (BHRF). Golden Heart Care is working on serving members post BHRF services which will allow for better quality in services since we have already assessed the individual on a 24 hour basis for months. GHC will provide data and detailed records of the day to day of the member while he/she is in a Golden Heart Care Residential Facility.

Golden Heart Care LLC will soon offer psychiatry services to members we serve and beyond! Golden Heart Care will be able to provide much better quality in psychiatry services by communicating effectively on a weekly basis with the residential team to help each individual reach their fullest potential at a much faster pace! This will eliminate a lot of miscommunication and error between home health, residential, hospitals, etc.

Golden Heart Care believes fully in the importance of promoting a healthy living and healthy habits. Golden Heart Care residential programs offer planned activities according to the members needs in each home. We develop activities such as personal training with our certified personal trainer (certified to work with the special needs community and experienced!), scheduled sporting events and community recreation involvement (special olympics).

A behavioral coach will work with our members to help them improve their lives with a focus on behavior and habit change. This coaching process includes: self-analysis, goal setting, Treatment Planning, and, most importantly, behavioral follow-through. Mentoring services can encourage and empower in personal development. Mentoring can help achieve personal growth and being helped to identify and correct gaps in generic skills and knowledge.

We dig deep into each individuals background to develop assessments that help each provider and agency after us!
We provide counseling services to each individual we serve.
We send out staff to the homes so that they can assess your loved ones behaviors, patterns and triggers. We then utilize ABC analysis, Incident reports to track each behavior.
If your goal is to be back with your family, our goal is to develop that bond and respect so that your family can have you home!
We have staff on shift 24 hours a day, 7 days a week!
We also provide 1:1 staffing if necessary.
We are very involved in your loved ones discharge planning. We provide resources to your next step, data on the individuals behaviors, strengths and limitations and more! We basically provide everything we have gathered to the provider(s).
When in a crisis, our goal is to do our best to provide Crisis Prevention Intervention. We will utilize outside crisis centers as needed.
We will provide medication education and we will monitor the effectiveness of each medication change. We communicate closely with your doctor and ensure you are in the right direction!
Family meetings allow us to provide up to date information regarding your loved one. Our goal is to communicate weekly!
Social events are important to developing relationships and communication skills!
We are HUGE believers in the term "health is wealth". A proper diet plan and exercise helps your mental health and physical well being so much!!
Mentorship is HUGE for everyone! It is important to have a mentor who can provide education and guidance to help you reach your end goals! We have mentors who meet with your loved one 1-3 times weekly at the home, in the community, at the gym, wherever!
Antecedent-based interventions can be used to decrease an identified interfering behavior and increase engagement by modifying the environment.
Cognitive behavioral intervention teaches learners to examine their own thoughts and emotions, recognize when negative thoughts and emotions are escalating in intensity, and then use strategies to change their thinking and behavior.
Self-management teaches learners with ASD to discriminate between appropriate and inappropriate behavior, accurately monitor and record their own behaviors, and reward themselves for appropriate behavior or use of skill.
SST refers to any adult-directed instruction in which social skills are targeted for improvement.
Differential reinforcement is an application of reinforcement designed to reduce the occurrence of interfering behaviors (e.g., aggression, self-injury, stereotypic behavior).
Exercise (ECE) can be used to improve the physical fitness of learners with ASD. In addition, exercise can be used to increase desired behaviors (time on task, correct responding) and decrease inappropriate behaviors (aggression, self-injury).
Prompting is an effective practice to increase success and generalizability of target skills or behaviors for learners with ASD.
A functional behavior assessment can be used when the intensity, duration, or type of interfering behavior creates safety concerns or impacts a person's development.
By using modeling (MD), the learner with ASD can acquire and generalize new skills/behaviors.
By using visual supports (VS), the learner with ASD might be able to process information easier and more quickly.

We have learned that a lot of these members do not like the homes they admit to. But we currently have no other options.

With our Show Low location of 40 acres, we plan to build a housing community of Behavioral Health Residential Home. We truly believe that our members would benefit from "Starting fresh" where it is so much more free! Our member's need animal services along with the services we already offer! We are seeking to take our services for your loved one who may be homeless o struggling to adjust to most homes he/she moves to.
Golden Heart Care plans to submit for contracting with the Department of Developmental Disabilities (DDD) to provide group home services, DTA services and in home care. We are also working on contracting with AZ Complete Care because we need to serve their members in Maricopa county and all other counties within Arizona.
Golden Heart Care will open outpatient services to it's members.
Golden Heart Care will soon find a way to provide step down options to our adults who have successfully completed programming and need the next step down opportunity!
