Through this program, Golden Heart Care provides valuable support to children and adults with disabilities in the convenience, safety and comfort of their own home. Services give much-needed relief for the primary caregivers and are essential in ensuring that members are able to remain in the home setting versus living in a long term care facility. Employees delivering care receive extensive classroom instruction and hands-on training in a living situation, in addition to member-specific training prior to placement in the family home.
Attendant Care/Personal Care
The Attendant Care Service is developed to attain or maintain safe and sanitary living conditions (personal and environmental), allowing an individual to remain in his/her personal home.
Our goal is to develop and implement a schedule/plan of care to ensure that the highest quality of life is achieved for the people that we serve. To complete this service objective, we typically focus on improving the following areas:
Respite
Our Respite Service provides short-term relief to a typical care provider and/or guardian. This service is designed to provide quality care to individuals in the event that their primary care provider is unavailable.
Our objective is to ensure that the individual needs are met in the absence of the primary care provider.
This service can be provided in individual's homes, various licensed community settings, group home, developmental home and nursing facilities.
Staffing needs are based on the individualized program and the needs of the individual being served. In-home respite is typically served at a one-to-one ratio unless there are other special needs children/adults in the home.
At Golden Heart Care our employees are available 24 hours a day, seven days a week, including holidays. We tailor our services to meet your specific needs. Scheduling is done on your terms. We pride ourselves on being flexible and attentive to your needs. All caregivers meet all State of Arizona requirements, have a valid Finger Print Clearance Card, a valid AZ Drivers License with a clean motor vehicle report and have successfully passed all of The Golden Heart Care training courses and background checks. Our dedicated caregivers are trained to respect you, your home, and your beliefs while affording you the chance to remain in the setting of your choice. This program gives people an alternative to assisted living or nursing homes. Our goal is to assist you to live life on your own terms.
Habilitation:
The Habilitation at Home service based out of the participants own residence. We provide outcome-based training to enhance personal skill level in the home and community. Skill training is designed to increase or maintain an individuals skills in the following areas.
Staffing needs are based on the individualized program and the needs of the individual being served.
The goal of the HAH program is to help you achieve a greater quality of life that promotes self growth and satisfaction.
All employees meet the State of Arizona requirements, have a valid Finger Print Clearance Card, and have successfully passed all of the Golden Heart Care training courses and background checks.
